To apply for financial support from Pacifica Family Fund, please complete the following steps:

Step One: 

Sign up for an Orientation at Pacifica Family Maternity Center. Orientations are held every other week. If you are unable to make any of the dates/times listed, please email and we can accommodate your schedule! 


Step Two: 

Attend an in-person Orientation at Pacifica Family Maternity Center. If you are medically eligible to receive midwifery care at the Pacifica Family Maternity Center, you can request an application form. You can also email to receive a digital application after attending the Orientation. The application form has a Financial Assistance Eligibility Table (see below also), so you can see what level of support may be available for your family, based on your annual household income.


Step Three:

When we have received your application, we will send you notice of receipt within 1 week. You will then receive a formal eligibility decision within the next few weeks.
